New Patient Registration
To Register at Springfield Surgery
We welcome new patients. In order to register please either click on the link below or call into the surgery anytime between 9 a.m. and 6 p.m. We ask that you avoid coming on a Monday as this is our busiest day. Please bring with you a form of photographic evidence and a utility bill with your current address on. We also need to know the name and address of your previous G.P.
- Complete a registration form online
(Please note: Your registration will not be complete until you visit the surgery and sign the printed form. You will need to bring a form of photographic ID and a utility bill showing your current address when you come in to the surgery to sign the form. You will then be given an information pack about the surgery and how to access our services.)
Once you have registered we will book you an appointment for a new patient medical. If you are on any medication we will book you an appointment to see one of our Doctors.
If you live in one of the villages surrounding Brackley (apart from Middleton Cheney) you will be able to use our Dispensary to obtain both repeat and acute medication.
